As of 1st April 2008, new child death review processes became statutory for the Local Safeguarding Children's Board and all partner agencies.

1. Introduction
There are two strands to the review process these are:
- A rapid response process in the event of any unexpected death
- A review process for all child deaths (any child 0-18 resident in the borough, irrespective of the place of death)
Both are likely to involve Children and Families at some stage if Social Workers have been involved directly with the child that has died or with other family members.
2. Rapid Response
Although it is unlikely (but not impossible) that a social worker would be part of a rapid response team required to respond at the time of death, if a child /family is known to them they will be contacted once the death is notified and asked to provide information for a case discussion. If the death is both unexpected and suspicious, there would be an investigation as per normal procedures alongside the child death process.
3. Child Death Review
If a child dies in a way that is not unexpected but a social worker has been involved with them or their family, once the death has been notified to Single Point of Contact, the social worker and their manager will be contacted. They will then be sent and asked to complete an audit tool, not all of which will be relevant to them. This will then be one of several audit tools submitted by each involved agency for consideration at Haringey's Child Death Overview Panel (CDOP), in order that a detailed picture of the circumstances can be built up and an evaluation made as to its preventability. Statistical information from each review will be passed to the London Safeguarding Children Board, which will produce an annual analysis of trends for the whole of London.
The Chair of Haringey's CDOP is the Deputy Director Children's Services; Sarah Peel is the Single Point of Contact, or SPOC, for the borough.
